WAIVER AND RELEASE OF LIABILITY

       In order to participate in the League and access your team's schedule you must fill out the Waiver Form below and create a Username and Password. Once you have done this, you will no longer need to fill out a waiver form each season! However, you will need to review your waiver information each season to make sure we have the most up-to-date information. In addition, after entering your Team ID on the next page you will only need your Username and Password to access your team's schedule for that season.
